Pharmacy Technician - Patient Care Coordinator

Position Summary

As the Patient Care Coordinator, you are responsible for coordinating enrollment and follow-up into Care Navigation Services for patients transitioning from acute care settings into the long term care environment through discharge to home. You will work closely with internal and external health care workers to promote excellent patient care and improve transition outcomes.
Hours: Monday - Friday, 9:00am - 5:30pm. You will:
Collaborate with pharmacy staff, nursing facility staff and physician offices.
Triage all incoming phone calls and faxes.
Enter new orders in a timely and accurate manner.
Scan, file and electronically store all patient discharge orders, prior authorizations and other related documentation.
Assist the pharmacist in identifying covered alternatives.
Initiate Prior Authorizations as directed by the pharmacist.
Work with facility staff to gather pertinent information needed to secure prior authorizations.
Fax and electronically submit completed prior authorizations to insurance plans for review.
Call insurance plans to follow-up on submitted prior authorizations.
Coordinate scheduling of prescription counseling between patient and pharmacist.
Follow all applicable government regulations including HIPAA.
To be successful, you will possess excellent communication and organization skills, as well as the ability to work independently, meet deadlines and be flexible. Strong computer skills in data entry and Microsoft Office programs are also critical to your success.

Preferred Qualifications
Minimum of one year full-time Pharmacy experience strongly preferred.
PTCB certification preferred.
Required Qualifications
Pharmacy Technician License/Registration with the Illinois Board of Pharmacy required.
Knowledge of general pharmacy terminology.
